Patient Guide

If you are Corporate patient, you have to submit the following Xerox Copies

1. Surgery schedule with date of surgery mentioned.
2. Patient's Insurance Card or E-Card
3. Employee PAN CARD and PASSPORT SIZE PHOTO
4. Identification Proof of Patient is mandatory (photo/age) - Aadhaar card.

If you are an Individual Policy Holder - you have to submit the following Xerox Copies

1. You should have completed three years in case you are applying for Caseless
2. Surgery schedule with date of surgery mentioned
3. Patient's Insurance Card
4. Current Year Policy + Previous Year Policy.
5. Identification Proof of Patient is mandatory (photo/age) - Pan Card, Aadhaar card.
6. Primary insured AADHAR CARD, PAN CARD and PASSPORT SIZE PHOTO

If you are Tamil Nadu Government Employee I Pensioner - You have to submit following Xerox Copies

1. Insurance Card & Aadhar Card
2. If Employee unmarried - Should produce unmarried certificate to apply for their dependents
3. In absence of Insurance card
Employee should provide Annexure 7 form, Aadhar card and current pay slip
Pensioner should provide Annexure 4 form, Aadhar card and optional form.

The following points are applicable to the entire above Insurance holder

• Any Change in Surgery Date, Surgery Procedures, and Room or Bed changes should be informed to Insurance Help Desk .
• In case surgery is cancelled under unforeseen circumstances the patient will have to bear the cancellation charges. Insurance provider will not bear the charges.
• Patient will have to pay Non-Medical expenses and co- payment whenever specified in authorization letter .
• Cosmetic surgery I Congenital disease /Yag Capsulatomy and Yag Iridectomy are not covered under cashless You can opt. for reimbursement.
